Transaction Requests for Current Employees

Managers can request employment transaction changes to existing employment records, such as; reappointments, time base changes, supervisor updates, and pay plan adjustments. Use this form to initiate a personnel transaction for Staff/MPP (not for student employees).

How to submit:

  1. Download the Personnel Transaction Request Form (PTRF).
  2. Managers must complete required fields 1 and 2.
  3. Attach justification with supporting documentations, if applicable.
  4. Submit to Human Resources.

Do not send form to budget or manager(s) for approval. The Employment, Talent Acquisition unit (ETA) in HR will review for compliance and route for required approvals. Once fully approved, HR will finalize processing and issue corresponding notices to employees and copy the employees Administrator.

Temporary to Permanent status Converts a temporary employee to probationary or permanent employment status, in accordance with applicable policies and approvals. • Current Organizational Chart

Time Base change Modifies an employee’s assigned time base.

Example: Change from part-time (20 hours per week) to full-time (40 hours per week), or from full-time to part-time.

Pay Plan change Adjusts an employee’s pay plan designation (e.g., 10/12, 11/12, or 12/12).

Example: Change from a 10/12 to a 12/12 pay plan, or from a 12/12 to an 11/12 pay plan.
