In very specialized cases when it is warranted and with well-documented medical justification, DRS can authorize the use of a formula bank/word bank for use during exams in order to assist this student with the recall of information.

A formula bank is a 4” x 6” index card or single page created by the student that includes any formula the student anticipates having difficulty recalling during a course test.  It cannot include any notes or outline format that would provide cues as to when a given formula should be used.  The instructor should have an opportunity, prior to the test, to review and sign off on it, being certain that it does not provide the student with an unfair advantage. 

A word bank is a 4” x 6” index card or single page created by the student that includes the specific, relevant vocabulary that the student anticipates having difficulty recalling during a course test.  It cannot include material that would provide the student with an advantage over the other students.  For example, it cannot include definitions or material such as proper names or place names or numbers/dates: this would change the test to an open notes test for this student.   The instructor should have an opportunity, prior to the test, to review and sign off on it, being certain that it does not provide the student with an unfair advantage.

The expectation is that the student will provide the Formula Bank/Word Bank to the instructor for approval at least 72 hours prior to the date of the exam and that the instructor sign-off on the Formula Bank/Word Bank indicating that the material approved does not fundamentally alter the requirements of the course by the student using this accommodation.
