Forms
This page contains the forms needed for the various services provided by Disability Resource Services. Forms are organized by the service they relate to and can be downloaded in PDF or DOCX (word). PDF forms are for filling out, then printing. DOCX forms are for printing and filling out by hand only. Forms are also available as a hard copy in our office. Screen reader users should download the PDF form.

Mac Users
If you are using a Mac to download the PDF version of any of our forms, please open and fill out the form in Adobe Reader. DO NOT USE PREVIEW. Using Preview will cause the changes to your PDF to appear "Invisible" when saved, and will take the form unusable. Unfortunately, this is a glitch that occurs with all Mac Users using Preview to fill out PDFs and is unavoidable.
