Safeway Pharmacy will be conducting a vaccine clinic to provide COVID-19 Boosters, initial vaccines, and flu shots to the faculty, staff and students ONLY of the University.
What to bring:
- Completed consent form (download via the appointment link)
- Medical and prescription card
- CDC vaccine card for 2nd dose and booster appointment
