New Staff Hire Documents

Please complete the following forms online, print, and bring with you to your sign-in appointment meeting with a Human Resources representative. You will have the opportunity to review the forms, ask questions, and add your signature and date as indicated on each form.  In some cases, you may not receive these documents until your first day of employment. Please note, your pay cannot be processed until you have completed the required forms.

*REQUIRED* Hiring Forms

    Oath of Allegiance & Declaration of Permission to Work for Persons Employed by the State of California - Review the entire document and sign.

    Form I-9, Employment Eligibility Verification - Please complete Section 1 ONLY.  If any boxes do not apply, do not leave blank, but write N/A. Bring this form and your original identification documents to your sign-in appointment with HR.  Please see acceptable documents listed (last page of the I-9).  You may bring one from List A OR a combination of one selection from List B AND one selection from List C. All documents must be original (no photocopies) and UNEXPIRED.​
     
  3. ​​​Employee Action Request -In section B, check "01 - New Employee" and complete sections as stated – C, E, F, G, H, I. 
     
  4. Employee Personal Data form – Complete all applicable items. 
     
  5. Designation of Person Authorized to Receive Warrants – The purpose of this form is for you to name a designee to receive your final paycheck in the event of your death.  The designee must be 18 years of age or older. 
     
  6. CalPERS Reciprocal Self Certification Form - Complete all applicable items.


    Additional *Required* Forms for Hourly-Intermittent and/or Emergency Temporary Staff ONLY:
     
  7. CSU Form SSA – 1945 Statement Concerning Your Employment in a Job Not Covered by Social Security - Please read, sign & date. 
     
  8. Retirement Plans for Part-time, Seasonal & Temporary Employees – This form is to determine your appropriate retirement plan; please check applicable statement, print name & sign.
    •  If you have checked “None of the above,” you will automatically be enrolled in the alternative retirement plan Department of Personnel Administration, Savings Plus Program (DPA-PST).

Optional Forms

  • The following forms are used to gather statistical data and are voluntary; therefore if you do not want to participate or if this does not apply to you, forms can be left blank:
  • Direct Deposit – If you are interested in receiving direct deposit, please fill out the form. It will take 30-60 days for your direct deposit enrollment to take effect.
