To accommodate the needs of CSU students, the English Placement Test (EPT) and the Entry Level Mathematics Test (ELM) will be administered at CSU Stanislaus on the dates listed below. If you plan to test at Stanislaus, you must register using this form and pay your fee when you register; all other campuses use the bubble registration form. Download the EPT/ELM Form.pdf.

These diagnostic/placement tests are mandated by an Executive Order of the CSU Chancellor. If you believe that you are exempt from testing contact the Records Coordinator in person (in Room MSR 120) or by phone at (209) 667-3264.

  • Alternate test arrangements may be provided for Saturday Sabbath observers and for disabled individuals who have been approved for special test accommodations.         
  • Disabled students should obtain a Special Request Form from this website, have it signed by Disability Services and attach it to this registration form along with their current semester class and work schedules.*        
  • Saturday Sabbath observers: please obtain a letter, on official letterhead, from an appropriate religious authority, and attach it with your class and work schedules to this registration form. You will be notified in writing of your test date.
    • Tests requiring special accommodations may not be on the actual test date. You will be notified in writing of your test date and time (that's why we ask for a class/work schedule).
    • Applications must be postmarked by the deadline above.
    • Candidates must present valid government ID on the day of the test (school ID cards are NOT acceptable – see information below).
    • If you prefer, you may deliver your form and payment to the Testing Office BY 3:00PM ON THE REGISTRATION DEADLINE (dates listed above).
  • Incomplete applications will be returned.
  • Applications postmarked after the registration deadline will not be accepted.
  • Specific reporting instructions (i.e. time and place) will be mailed approximately one week before the test.
  • It is your responsibility to obtain the reporting information.
  • If you have not received your report letter by the Wednesday before the test, please pick up your reporting information in the Testing Office, MSR 180.
