Shredding Service Request Form
Support Services provides campus departments with centralized shredding services. The contract for services is arranged for, and maintained by, Support Services. Departments request the type of service desired by entering the following information. This form automatically submits the request to the Support Services department. You will be contacted to confirm your service request and be provided with cost information. A monthly recharge amount will be entered into PeopleSoft finance based on the service provided.
