CONFIDENTIAL PEER EVALUATION FORM – GROUP CASE & GROUP INTERNET EXERCISES

Each team member will rate him/herself and the other team
members on the following criteria. The percentages should average to
100%. For example, if all members participated equally, then each gets
100%. If some of the group members contributed more or less, then the
ratings might be (110%, 100%, 90%, and 100%) or (150%, 125%, 100%, & 25%)
or whatever as long as it averages to 100. These evaluations are to be
handed directly to Dr. Williams in order to maintain confidentiality.
Each group member will hand in one peer evaluation for the group case after you
have finished your in-class case presentation and another peer evaluation for
all of the Internet Exercises combined when Internet Exercise 5 is due. Evaluations by each team member must be
received by Dr. Williams before the graded case can be returned.
