Important forms for completing the Multiple and Single Subject Teaching Credential Admission Process.
| FORM I - Statement of Intent | Information on how to write your statement of intent. Must be one to two pages long. Not required for CSU Stanislaus Liberal Studies Majors. |
| Form IIa - Letter of Recommendation | This form is used for students who are applying to the Multiple Subject Credential Program. |
| Form IIb - Letter of Recommendation | This form is used for students applying to the Single Subject Credential Program. |
| Form IIc - Letter of Recommendation | This form is used for students applying to the Preliminary Education Specialist Credential Program. |
| Form III - Verification of Early Field Experience | This form can be used to submit verification of experience hours. You can also submit letter from school indicating number of hours completed. |
| Form IV - Technology Proficiency Requirement | This form is used to demonstrate proficiency with technology. Not required for CSU Stanislaus Liberal Studies Majors. |
| Form V - Subject Matter Preparation Verification | This form is used for Single Subject applicants. Please have the advisor in the subject area sign and turn in with the application. |
| Form VI - Ethnic Identity (optional) | This form is optional. Please submit with your credential application. |
| Form VII - Prerequisites/Co-Requisites | This Form is used for students applying to the Multiple Subject and Single Subject Credential Program. |
| Certificate of Clearance Form 41-LSa. |
Applicants must submit fingerprints via “livescan” and be “cleared” before working with students. This form will give you instructions on how to apply for the certificate of clearance. Not required if you submit a copy of a valid teaching credential/ permit or child center permit. |
