I.D. Clearance
New Requirement
Effective immediately all students applying to Credential Program needing to submit the Character and Identification Clearance (blue form 41-CIC), must also submit the processed Livescan Form (41-LS). The Livescan Form (41-LS) must have the ORI number of A0281 listed in the top right corner and the Level of Service must have both DOJ (Department of Justice) and FBI checked in the Applicant Personal Descriptor Information section on the form . This form must be picked up from the Credential Processing Center in Turlock (Demergasso-Bava Hall 303) or at the Administration Office in Stockton Center (Acacia Building). The form may also be available at local school districts or county offices of education but must carry the ORI number A0281 and have DOJ and FBI checked or it will be invalid and a correct processed form will be required.
If you have already completed hard copy fingerprint cards, live outside of California, or reside in California counties which do not have access to Livescan facilities, contact:
