Planning a camp or clinic on campus? There are special considerations and guidelines to manage the risk and insurance for sports and academic camps held at CSU Stanislaus.
Here is what you will need to do in order to plan a safe and successful camp or clinic:
Fill out the Camp / Clinic Risk Assesment.pdfand return it to the University Police Department Attn: Safety and Risk Management, at least 14 calendar days PRIOR to the event start date
Notify Risk Management of all adult staff members. We will check the fingerprint clearance database, and let you know which persons still need to be fingerprinted. You will be contacted regarding any background clearance procedures necessary.
Use the appropriate Participant Release/Informed Consent form: (Non-Athletic Camp Clinic Release of Liability.pdf | Sport Camp Participant Release.pdf) A Release form must be received from EVERY camp participant. Minor age
(<18 years) participants must have a parent/guardian complete and sign the form. Your department must keep the original signed documents on file for one (1) year after the camp is completed.
Fill out a roster.pdf and return it to Safety and Risk Management after the camp is completed for compliance with the camp insurance coverage.
If you plan to have food at your camp, and it is not provided by Campus Dining, then you must fill out and submit a Temporary Food Permit.pdf form at least 14 calendar days PRIOR to the event start date.
